Family Support Services

Family Support Services is a program to help families keep their developmentally disabled child or adult living at home who are not currently receiving other Medicaid Waiver services.

Families who are interested in having their child return from an out-of-home placement are also eligible for our program. The goal of our services is to provide families with assistance to help alleviate some of the common stresses experienced by families who have a member with a disability.

 

The Family Support Services Program is advised by a council made up of family members receiving services and community stakeholders. Current members are Kelly Durbin, Tamara Durbin, Louise Lopez, Michele Van Sice & Amy Roys. Anyone interested in becoming a member of the Council is encouraged to contact Melissa Dassaro by phone, mail or by email.

Below you will find our electronic Family Support Plan Application and Family Needs Questionaire that you can download, complete and send into your Case Manager. Both the completed application and questionaire must be returned.

You can also print out the "Families Most In Need Checklist" which is criteria mandated by the legislature that may assist you in completing the application. For your review, please view/print a copy of "Family Support Services Basic Rights" as well. Feel free to contact your Case Manager if you would like help in filling out the application or questionaire.
